The following questions are being asked of you so that the right puppy and the right placement of each puppy is made by bringing the various requirements of both the puppy and it’s new owner(s) into perspective, BEFORE a choice is made. Please answer the following questions as completely as you can, and return this form via email when you've finished.

- Who will be the primary care giver?
- Where will the puppy be kept during the night?
- Where will the puppy be kept during the day?

- How many hours on the average will the puppy be left alone?
- Will there be someone available to feed and exercise the puppy during the day?
- Do you have the time to meet the demanding needs of the puppy/dog? Time for feeding, training and exercise? Please remember that Golden Retrievers are sporting dogs that need a lot of mental and physical stimulation.
- Have you ever owned a dog before?
- How long did your last pet live? What were the circumstances of it’s death?
- Do you have any other animals?
- Do you have any children? If yes, what are their ages? How would they be instructed in the care of the dog? The average weight of a Golden is anywhere between 55 lbs to 75 lbs and an excited teenage dog can easily knock over a small child without meaning to.

- What type of personality/activity level are you looking for in a golden?
- How many hours a day will the puppy be kept outside?
- How will the puppy be confined when it is outside?
- Do you have a fenced yard or suitable pen?
- Have you ever house trained a dog before?
- Will you be attending any training classes?
- If yes, at what age will you start, which activities or competitions are you interested in?
- Have you ever returned a pet to the breeder? If so, what were the circumstances?
- Have you ever given a pet away? If so, what were the circumstances?
- Have you ever taken a pet to a pound or shelter? If so, what were the circumstances?
- Are you interested in showing the dog in the conformation ring?
- Are you interested in competition in performance events such as obedience, agility , hunting, herding, etc.?
- Do you intend to breed the dog?
- Do you understand the difference between a limited and a full registration?
- Do you feel we can maintain a long-term relationship to discuss the health and welfare of your Golden Retriever?
- Do you agree to return this Golden Retriever to the breeder in the event you are no longer able to care for the dog, regardless of age?
- Do you plan on registering your new puppy with the AKC?

On approval of this application a deposit of $200.00 will be required to hold your puppy. This deposit is refundable up until the puppy is 8 weeks old. This application, or an acceptance of a deposit does not guarantee a puppy from the current litter. All puppy placements are at the discretion of Oxford Goldens.
